La hipertensión intracraneal idiopática (HII) se define como el aumento de la presión intracraneal en ausencia de masa intracraneal o hidrocefalia obstructiva. La cefalea, los trastornos visuales y el papiledema son los rasgos característicos de este síndrome, pero los pacientes con IIH también pueden experimentar tinnitus y pérdida de audición. Presentamos un caso de una mujer joven con pérdida auditiva bilateral fluctuante y tinnitus pulsátil, que finalmente fue diagnosticada mediante una punción lumbar de hipertensión intracraneal idiopática tras un ingreso por cefalea. La respuesta al tratamiento con acetazolamida hizo que la audición se normalizara. Los otorrinolaringólogos deben estar familiarizados con esta patología ya que se puede manifestar con una variedad de síntomas que se ven habitualmente en su práctica clínica.
Idiopathic intracranial hypertension (IIH) is defined as increased intracranial pressure in the absence of intracranial mass or obstructive hydrocephalus. Headache, visual disturbance and papilloedema are the characteristic features of this syndrome, but patients with IIH can also experience tinnitus and hearing loss. We present a case of a young woman with fluctuating bilateral hearing loss and pulsatile tinnitus who was finally diagnosed with idiopathic intracranial hypertension through a lumbar puncture after a headache income. Response to treatment with acetazolamide made the hearing normalize.
Otolaryngologists should be familiar with this condition as it can be presented with a variety of symptoms commonly seen in clinical practice.
