  Definición La colestasis extrahepática es un problema clínico frecuente y puede ser la manifestación de múltiples enfermedades con un tratamiento y pronóstico muy diferente.

  Métodos diagnósticos La anamnesis y la exploración física pueden orientarnos enormemente en el diagnóstico, y la ecografía abdominal debe ser la primera exploración a realizar. En casos en que esta sea indeterminada o normal pero la sospecha clínica sea alta, debe completarse el estudio con otros métodos de imagen. La ecoendoscopia y la colangiorresonancia son exploraciones con una rentabilidad diagnóstica alta y comparable en este escenario. La CPRE y la CTPH son exploraciones invasivas que ofrecen posibilidades diagnósticas y terapéuticas en los pacientes con obstrucción de la vía biliar.

  Definition Extrahepatic cholestasis is a common clinical problem and can be the manifestation of numerous diseases with very different treatments and prognoses.

  Diagnostic methods The case history and physical examination can help enormously in directing the diagnosis. Abdominal ultrasonography should be the first examination performed. For cases in which the results of this examination are undetermined or normal but clinical suspicion is high, the study should be completed with other imaging methods. Echoendoscopy and cholangioresonance are two comparable examinations with high diagnostic yields in this scenario. Endoscopic retrograde cholangiopancreatography and transparietal hepatic cholangiography are invasive examinations that offer diagnostic and therapeutic possibilities in patients with bile duct obstruction.
