  El shock es una situación hemodinámica frecuente y altamente letal. De etiología multifactorial, engloba un amplio espectro clínico que obliga a elaborar un adecuado diagnóstico. La corrección de las alteraciones de la precarga ventricular, contractilidad, postcarga y oxigenación sistémica serán los pilares básicos para el manejo. La perfusión tisular inadecuada que no responde al tratamiento inicial debe llevar a valorar un abordaje agresivo y dirigido por objetivos. A su vez, la identificación de la causa subyacente y el abordaje terapéutico precoz son los objetivos diana para evitar complicaciones potencialmente devastadoras que supongan la instauración de un fracaso multiorgánico.

  The shock is a common and highly lethal hemodynamic status. Multifactorial etiology encompasses a wide clinical spectrum that forces develop a proper diagnosis. The correction of changes in ventricular preload, contractility, afterload, and systemic oxygenation will be the cornerstones for management. Inadequate tissue perfusion that does not respond to initial treatment, must take an aggressive approach to value and goal-directed. In turn, the identification of the underlying cause and early therapeutic approach are the target objectives to prevent potentially devastating complications involving the establishment of multiple organ failure.
