Estudio clínico controlado aleatorizado para evaluación del Emdogain como terapia adjunta en el tratamiento de recesiones gingivales/Randomized controlled clinical trial for evaluation of Emdogain in adjunctive therapy in gingival recession treatment

  • ANTECEDENTES: publicaciones previas han demostrado que las proteínas derivada de la matriz del esmalte tienen la habilidad de mejorar los parámetros . OBJETIVO:comparar clínicamente los resultados obtenidos con relación a profundidad de la bolsa, nivel de inserción clínica al aplicar el Emdogain, más injerto de tejido conjuntivo (ITC)con colgajo posicionado coronal (CPC), con ITC con CPC como tratamiento único, en recesiones ginigivales tipo I y II de Miller para verificar si con el Emdogain mejora el cubrimiento de las recesiones (CR).METODOS: Este estudio clínico controlado a boca dividida fué realizado en 16 recesiones gingivales bilaterales clase I de Miller, seleccionados al azar (grupo experimental: Emdogain e ITC con CPC). El diente contralateral fue asignado al grupo control (ITC con CPC).Se realizaron medidas al inicio y a tres meses del margen gingival, profundidad al sondaje, nivel de inserción clínico y cantidad de encía adherida. Los datos fueron analizados con prueba de U de Mann-Whitney, Fischer y Wilcoxon, se tomó una significancia de p0.05. Se incluyeron pacientes con recesiones clase I y II de Miller bilaterales (3 mms), índice de placa de O´Leary 30%;ausencia de sangrado al sondaje.Se excluyeron pacientes con enfermedades sistémicas, aquellos con ingesta de medicamentos,pacientes con enfermedad periodontal activa,abfracciones,grietas, irregularidades o restauraciones en el área a tratar, que por su profundidad no pudieron ser removidas. RESULTADOS: ambos tratamientos resultaron ser efectivos para el CR.El pocentaje promedio de CR en el grupo control fué del 100% y para el grupo experimental fué de 87.5%. Aunque hubo mejores resultados en el grupo control, las diferencias entre ambos grupos no fueron estadísticamente significativas(p=0.077). CONCLUSIONES: la aplicación de Emdogain no mejora significativamente el resultado clínico al utilizarlo en conjunto con ITC y CPC en el tratamiento de las recesiones gingivales.Palabras claves:recesión gingival, proteínas derivadas de la matriz del esmalte, injertos, tejidos conjuntivo.Area temática: PeriodonciaBACKGROUND: previous studies have demonstrated that enamel matrix derivative has the ability to improve clinical parameters.OBJECTIVE: to compare the results obtained in relation con probing depth, clinical level attachment and keratinized tissue when applying Emdogain plus connective tissue graft (CTG)with coronal advanced flap (CAF) like unique treatment, in gingival recessions Miller´s type I or II to verify if Emdogain improve coverage of gingival recessions. METHODS:This randomized controlled clinical trial, split mouth, was made in 16 sites with ginigival recessions bilateral Miller´s class I , selected a random for de experimental group (Emdogain and SCTG). The contralateral tooth was assigned to the control group (SCTG). Measurements at the beginning and three months included:ginigival margin,probing depth, clinical attachment level and keratinized tissue.The data were analized with U Mann-Whitney test, Fischer test and Wilcoxon text ; and was taken a significance of p=0.05. RESULTS: results for both the test and control groups were similar for all measured clinical parameters.Both treatments showed to be effective  with root coverage (RC).The percentage average of RC in the group control was 100% and for the experimental group was 87.5%. Although there were better results in the control group, the differences between both groups were statistically significant (p=0.077). CONCLUSIONS: based on the results of this investigation, the addition of Emdogain to SCTG with CAF it significantly does not improve the clinical results.Key Words: gingival recesions,enamel matrix proteins,grafts,connective tissueThematic fiel: Periodontics
