Objective: To evaluate users’ adherence to and satisfaction with an electronic pharmacological support tool for assistance in prescribing medication in the discharge reports completed.
Method: Transversal descriptive pilot study carried out, during April 2013, in one medical and one surgical unit of a General Hospital. The developed tool provides the physician responsible for completing the discharge report, the patient’s home medication. It includes possible pharmacological recommendations and permits its transmission to the primary care physician. Adherence and how the physicians had used the tool were calculated by records extracted from the program supporting it and by reviewing the discharge reports. The frequency of pharmacological recommendations was measured considering that it had been «accepted» if the physician had made the modifications suggested or if they were included in the discharge report. Satisfaction with the tool was measured by a survey containing 17 items answered by a Likert scale.
Results: 215 patients (105 from medical service and 110 from surgical service) were included in the study. Discharge reports made by 41 physicians were evaluated.The degree of overall use of the tool was 84.2%, significantly higher adherence by medical service than by surgical (p <0.01). A total of 133 pharmacological recommendations were made (36.8% of it were accepted). 70.7% of surveys were returned.
The physicians reported a high level of satisfaction on all survey items.
Conclusions: The developed tool is very useful as confirmed by the feedback obtained by the physicians. Pharmacological recommendations offered can be used for improving communication between different levels of health care
