Resumen de Estudio neuroinmunológico en un paciente con manifestaciones neuropsiquiátricas en el lupus eritematoso sistémico

  Las manifestaciones neuropsiquiátricas del lupus eritematoso sistémico en su debut pueden resultar de difícil diagnóstico en ausencia de otros elementos de su cuadro clínico. El reibergrama, o gráfica de las razones de Reiber, son diagramas en los que básicamente se analiza de forma integrada la funcionalidad de la barrera sangre- líquido cefalorraquídeo y la síntesis intratecal de una proteína determinada. Se presenta un paciente con manifestaciones neuropsiquiátricas que en el curso de la enfermedad se identifica como un lupus eritematoso sistémico con afectación del sistema nervioso central y respuesta favorable al uso de esteroides endovenoso. Se reflejan exámenes complementarios que orientaron a este diagnóstico y se establece el patrón de síntesis intratecal de inmunoglobulinas mayores, a través  del reibergrama. Constituye este estudio el primer reporte sobre la respuesta neuroinmunológica en un paciente con manifestaciones neuropsiquiátricas en el  lupus eritematoso sistémico.

  Manifestations neuropsychiatric in Systemic lupus erythematosus debut can result of difficult diagnostic in absence of other elements of its clinical manifestations. Reibergram or Reiber´s quotient diagram are used in order to analyse the blood-cerebrospinal fluid barrier function and the proteins intrathecal synthesis. It presents a patient with manifestations neuropsychiatric that in the course of the illness is identified like a Systemic lupus erythematosus with affectation of the central nervous system. The patient showed a favourable response to use of steroids intravenous. It reflects complementary tests that oriented to this diagnostic and is established the intrathecal synthesis pattern of the major immunoglobulins using the reibergrama. This study constitutes the first report of the neuroimmunological response in a patient with neuropsychiatric manifestations in Systemic lupus erythematosus.
