    En este trabajo damos a conocer el primer testimonio de una mastectomía por cáncer de mama realizada en América, concretamente en Aguascalientes (México), en el siglo XVIII. Esta intervención ha quedado registrada en un exvoto anónimo en el que la señora Josefa Peres Maldonado da gracias al Cristo Negro del Encino y a la Purísima Concepción, o Virgen del Pueblito, por el buen resultado obtenido. La operación fue realizada por el médico y cirujano francés Pedro Maillé, con el auxilio de frailes del convento-hospital de San Juan de Dios. Revisamos la historia del tratamiento quirúrgico del cáncer de mama, la estructura pictórica del documento, la técnica quirúrgica propuesta por el Real Colegio de Cirugía (Nueva España) en aquella época y la asociación entre este acontecimiento y su tiempo, como una muestra del impacto de la Ilustración en la Nueva España.

    In this paper we present the first evidence of a mastectomy for breast cancer in America, performed in Aguascalientes, Mexico, in the eighteenth century. This intervention was recorded in an anonymous ex-voto in which Mrs. Josefa Peres Maldonado thanks the Black Christ of Encino and the Immaculate Conception or Virgin of the village for the good results obtained. The French physician and surgeon Peter Maille performed the operation with the help of friars of the convent-hospital San Juan de Dios. We review the history of the surgical treatment of breast cancer, the pictorial structure of the document, the surgical technique proposed by the Royal College of Surgeons (New Spain) at the time, and the association between this event and its time as an example of the impact that the Age of Enlightenment had in New Spain.
