Intervención farmacéutica en pacientes de neurología y cirugía

    • Objective: To analyze the pharmaceutical interventions generated during the pharmaceutical validation of pharmacotherapeutic prescriptions in an integral unit-dose system in surgery and neurology patients. Methods: A prospective study carried out for over 40 days in the year 2003 on patients admitted for digestive surgery, urology, neurosurgery, neurology and short-stay surgery (176 beds). The interventions made, the care (PVC) drug related problems (DRP) and the results of pharmaceutical actions (PA) were all analyzed. Results: There were 744 admissions and 5,041 hospital stays. 100% of the 6,782 prescriptions were validated, without counting the infusion solutions. The distribution of the 359 interventions made by category was: Indication. Unnecessary drug: 28 (7.8%). Effectiveness. Inadequate drug: 3 (0.8%). Effectiveness. Under-dosage: 11 (3.1%). Safety. Adverse reaction: 5 (1.4%). Safety. Over-dosage: 18 (5%). Drug not included in Formulary: 226 (63%). Clarification due to illegible or incomplete prescription: 68 (18.9%). Of the interventions of PC, 80% sought to optimize the treatment, 17.5% preventive and 2.5% educational. 99.2% of the PC interventions were accepted. Conclusions: The recording and evaluation of the pharmaceutical interventions enable the identification of action areas. With a greater involvement of the pharmacist in the unit, more initiatives are taken and there is a greater possibility of increasing the quality of the interventions made
