Introduction: There is a great variability in guidelines recommendations about pemetrexed use in Non-Small-Cell-Lung-Cancer (NSCLC) depending on the area in which they are consulted. This study evaluates the adherence of pemetrexed use in Andalusia (Spain) to recommendations of different guidelines. Method: Retrospective-descriptive study. Adult patients with metastatic/advanced NSCLC who received pemetrexed during the first term of 2010 and the last term of 2011 in Andalusia were included. Data collected: Andalusian health area, pemetrexed indication and adherence (yes/no) to guidelines recommendations. Results: 277 patients from 17 hospitals were included. In 2010, 10.28% of pemetrexed prescriptions were consistent with local guidelines, 75.7% with national guidelines and 80.37% with international ones. In 2011, 13% of pemetrexed prescriptions were consistent with local guidelines, 75% with national guidelines and 84% with international ones. Conclusion: The adherence of pemetrexed use in Andalusia to national and international recommendations is higher than adherence to regional ones
