Resumen de Comprehensive information programme on discharge for patients with liver transplantation

  • Objectives: Analysing the results of an information programme on discharge of patients with liver transplantation. Improving care quality. Method: Each patient was interviewed about their medical record twice. Data was obtained in order to correct reconciliation problems, and a report on the prescribed medication was written, with drug images and information as well as recommendations on hygienic and dietary measures. Results: Since April 2010, 87 patients were included. The average number of drugs prescribed at discharge was 10.7. 12 medication errors were detected. A satisfaction survey was conducted among 74 patients and the result was 9.5 out of 10. Conclusions: Pharmacist interventions managed to avoid medication errors and were able to obtain direct economic savings. Patients feel that the activity undertaken by the pharmacist has made them more aware of their treatment
