A retrospective study, since July, 2000, to December, 2003, in which the prescriptions of drugs not included in the hospital's formulary (DNIF) are reviewed. The objectives of this work are: to describe the therapeutic groups and the medical specialties that mostly prescribe DNIF, to analyze the result of the pharmaceutical interventions being made and the achieved savings in the acquisition of these drugs. 3,866 prescriptions of DNIF were received. 70% of the active principles corresponded to five therapeutic groups. The most prescriptive medical specialties were: Internal Medicine, Neurology, Neurosurgery, Cardiology and Cardiovascular Surgery. In 34.2% (1,322) of the received prescriptions, the prescribed DNIF was not administered, being offered another therapeutic alternative. The introduction of a therapeutic exchange program and the pharmaceutical intervention have achieved an average monthly saving in the acquisition of DNIF of 1,172 euros. Moreover, they have allowed the patient to receive the most rational treatment in the least possible time. It is necessary to establish therapeutic exchange programs through the Pharmacy and Therapeutics Committee and promote the consensus in the drug selection process between primary and specialized care
