Nausea and vomits in the postoperative (NVPO) produce an additional cost of the surgical process and are, after pain, the greatest concern for the patient that undertakes surgery. Avoiding their appearance results in cost savings for the managers and a greater satisfaction for the patients. In an isolated form, no antiemetic is capable of resolving the problem, and the routine use of such drugs on all surgery patients is not cost-effective. To face the NVPO, we have to evaluate the risk, by means of validated scales, reduce it whenever possible, with non-pharmacological interventions, and, if still the possibilities of its appearance exceed 10%, use a multimodal approach with 1, 2 or 3 antiemetics of a different action mechanism, according to the probability of their appearance. We review the efficacy of the diverse antiemetics, alone or associated, and the reliability of the risk scales, attempting to control NVPO in a cost-effective way
