Diseño y explotación de un programa informático en agenda electrónica para el registro de las intervenciones farmacéuticas

  • Objective: To design a personal digital assistant (PDA) database to record and exploit the activities of a 4 year pharmacy resident during a stage in Internal Medicine Department. Method: A database was designed for PDA from HanDBase® Desktop 3.0 software. The database records patient characteristics (demographical and clinical) and pharmaceutical interventions for each patient. Data from medical records and medication profiles were recorded in a PDA before to perform the clinical round with the physician. Interventions were recorded from November 1 2004 to March 15 2005. Results: During this period 104 patients were recorded. The main causes of hospital admission were: respiratory infections, COPD exacerbation, dyspnea, cerebrovascular accident, etc. 326 interventions were made with a mean of 3.13 per patient. The main categories for intervention were therapeutic exchange (n = 57, 17.5%), drug not included in the formulary (n = 43, 13.2%), drug information < 5min (n = 32, 9.8%), sequential antibiotic therapy (n = 29, 8.9%), dose, interval and dosage form changes (n = 28, 6%) and monitoring recommendation (n = 24, 7.4%). Discussion: A simple, fast and complete recording was made with the designed database eliminating paper and systematizing daily practice. We obtained an important increase of interventions number regarding to the previous year
