Resumen de Pharmaceutical care of patients with polypharmacy in primary care

  • Objective: Identify and resolve the Negative Outcomes associated with Medication (NOM) on polymedicated patients, belonging to a medical group from the Health Center «Salvador Caballero» in Granada. Method: A quasi-experimental study without control group. The Dader Method, adapted to the primary care area, was used; each patient was evaluated with his doctor in order to identify the NOM. Result: 47.4% (95% CI, 37.34-57.71) of the treated patients had some negative outcome associated with Medication. The most prevalent NOM were those relating to safety, 24.24% (95% CI, 16.19-33.89). We intervened in 51% of cases and 91.67% of those cases were resolved. 78.7% of the total identified NOM were avoidable. In 19% of patients interviewed after the demand for health care the NOM was the cause of medical consultation
