Individualización posológica de ciclosporina en pacientes con trasplante renal: Aceptación del informe farmacoterapéutico

    • The objective of the present study is to evaluate the pharmacotherapeutic report developed during the posologic individualization process of cyclosporine in patients with kidney transplant. Within a population of 29 patients receiving kidney grafts and being treated with cyclosporine (CsA), it was evaluated the concordance between the pharmacist's recommendation and the physician's decision in reference to the modification of the posologic pattern, the adherence of the prescribing physician to the recommended daily dose of CsA and the proposed periodicity for the pharmacokinetic follow-up. There were also investigated, as possible prognostic factors associated with the acceptance of the pharmaceutical recommendation, the weight, the age, the sex, the post-transplant time, the recommended daily dose, the blood concentration of CsA, the serumal creatine and the recommendation of modifying the posologic pattern of CsA that patient was receiving. The concordance between the pharmacist's recommendation and the physician's decision shows evidence that the 71% (IC 95%: 69 to 71%) of all recommendations about the change in the posologic pattern are accepted. The supratherapeutic concentration of CsA and the recommendation of a dose inferior to 3 mg/Kg during the two first post-transplant months are prognostic factors independent of the acceptance of the recommended daily dose of CsA. The only prognostic factor associated with the acceptance of the recommended associated with the acceptance of the recommended periodicity of the CsA concentrations is the post-transplant time superior to two months
