Glucarpidase rescue in patients with high-dose methotrexate toxicity

    • Objective: To describe the case reports wherein delayed methotrexate elimination required management of glucarpidase. Method: Review of the patient's medical records who received glucarpidase between the years 2010 and 2012. Results: Four patients received a single dose of glucarpidase (50 IU/kg) after presenting delayed methotrexate elimination resulting in reduction of the methotrexate plasma concentrations and immediate decline of serum creatinine levels and hepatic enzymes in the patients. No adverse reactions were observed. Conclusion: Glucarpidase is an effective drug for the treatment of induced toxicity associated to delayed methotrexate elimination when hydration and folinic acid rescue are not sufficient
