Clase III ósea con desviación mandibular, desórdenes temporomandibulares y múltiples problemas dentarios asociados

    Nos encontramos ante una paciente con una maloclusión ósea de clase III mixta, con un prognatismo mandibular y una hipoplasia maxilar que se refleja sobretodo en sentido transversal. Funcionalmente hay una clara y marcada desviación a la izquierda con desplazamiento de la línea media inferior. La paciente presenta signos y síntomas relacionados con desórdenes temporomandibulares. Dentariamente destacan una clase II molar y la mordida cruzada posterior bilateral. Lo que hace más interesante a este caso es la asociación de problemas dentarios añadidos. Entre ellos: la inclusión del 15 y la grave destrucción de los primeros molares inferiores permanentes por caries. Durante el tratamiento sufrió un golpe en el 12 con resultado de fractura entre el tercio medio y gingival. Realizamos un tratamiento ortodóncico consistente en la colocación de aparatología fija en ambas arcadas, con quadhelix para la expansión del maxilar y extracción de 15, 25, 36 y 46 (con cierre de espacios).

    We are before a patient with a mixed skeletal malocclusion of class III, with a prognatic mandible and a hipoplasic maxillary reflected overalls in traverse sense. Functionally there are a white and marked deviation to the left with displacement of the inferior half line. The patient presents signs and symptoms related with temporomandibular disorders. Dental highlights a class II molar and the bilateral later crossed bite.

    That makes more interesting to this case it is the association of dental problems added. Among them: the inclusion of the 15 and the serious destruction of the first permanent inferior molars for cavity. During the treatment it suffered a blow in the 12 with fracture result between the gingival and medious third. We carry out a orthodontic treatment consistent in the placement of fixed appliances in both arcades, with quadhelix for the expansion of the maxillary one and extraction of 15, 25, 36 and 46 (with closing of spaces).
