    • In 2011, the authors of a report issued by the American Medical Association (AMA) concluded, �We still know very little about patient safety in the ambulatory setting and next to nothing about how to improve it.� And while improvements in outpatient safety remain elusive, this past August, researchers from the US Department of Veterans Affairs (VA) reported that they could at least identify some root causes that lead to delayed diagnosis and treatment, which increase the safety risk for these patients (Davis Giardina T et al. Health Aff [Millwood]. 2013;32[8]:1368-1375).
