Objective: To describe a case of neonatal hyperekplexia as well as the preparation of an oral clonazepam solution for its treatment by the pharmacy department. Methods: Starting from a commercial 2.5 mg/ml clonazepam solution, a 0.01 mg/ml clonazepam solution is prepared by means of serial dilutions. Such solution is dispensed to the patient at the hospital as well as outside in an external patient regime. A follow-up of the patient's evolution during the treatment takes place. Results: The initial dosage of 0.025 mg/kg/day produced not only a quick improvement on the patient, but also excessive somnolence. By adjusting the dosage to 0.01 mg/kg/day, it was observed an excellent tolerance and clinical evolution, reaching the point of medication removal six months from the beginning of the treatment.
