Objective: To describe the methotrexate monitoring in serum and cerebrospinal fluid (CSF). A case report of medulloblastoma. Method: Methotrexate levels in CSF were monitored before the first intra ventricular administration on day 1 of each week and 24 hours after for the rest of administrations. The methotrexate serum determination was performed at 26, 30, 48 and 72 hours on initiating infusion. We applied the Abbott® TDX fluorescence polarized Immunoassay Analyzer system in both cases. Results: The mean level of methotrexate in monitored CSF 24 hours after intraventricular administration exceeded the value of 1×10-6 M, reaching therapeutic objective, without exceeding toxic levels (5×10-6 M). Conclusions: Methotrexate monitoring in serum and CSF improves the efficacy of treatment and reduces adverse effects. This activity enhances the integration of pharmacy services in healthcare activity of Oncology units providing direct pharmacotherapy guidance
