  • Objective: To assess pharmaceutical interventions at an Intensive Care Unit (ICU) in the period from April 2010 to April 2011. The implementation of an assisted electronic prescribing (AEP) program and consultations to the pharmacist were included as secondary objectives. Method: A Drug-Treatment- Monitoring (DTM) database was designed. Clinical course and pharmaceutical interventions were recorded in the DTM database application. Halfway through the study period, training in electronic prescription was provided to the ICU staff. Results: Prescribed medication was reviewed in 87 patients, with at least one pharmaceutical intervention in 26.4% of them (98% acceptance). Anti-infectious drugs were the ones most commonly involved in DTM (60.5%). Electronic prescription was used for 75% of ICU admissions after its implementation. ICU staff performed 18 consultations to the pharmacist. Conclusions: The program to record the clinical course and pharmaceutical interventions is an effective tool for DTM. The high acceptance of pharmacist interventions shows the added value of pharmacists in the multidisciplinary ICU team.
