Objective: To compare error rates in the dispensing of solid oral medications as conventional unit-dose and improved unit-dose forms (Onedose®) over a two-month period. Method: Prospective observational study to record error rates in the preparation of medication carts in a hospital pharmacy service for omeprazole 20 mg capsules and acetaminophen 1 g tablets. Results: We reviewed 2,669 drawers with omeprazole 20 mg capsules and 568 drawers with acetaminophen 1 g tablets in conventional unit-dose forms, and 2,609 drawers with omeprazole 20 mg capsules and 795 drawers with acetaminophen 1 g tablets in improved unit-dose forms. The dispensing error rate for omeprazole was 1.01% in the conventional form and 0.27% for the improved unit-dose form. For acetaminophen, the dispensing error rate was 1.06% for the conventional form and 0.13% for the improved form. Conclusion: We observed a significant reduction in dispensing errors during the preparation of medicine carts when the improved unit-dose form was used
