La noción de responsabilidad subjetiva en contextos de intervención hospitalaria

    El objetivo de este artículo es problematizar los usos e implicancias prácticas de la noción de �responsabilidad subjetiva�, utilizada por el psicoanálisis como herramienta para conceptualización terapéutica. Se realizó un estudio cualitativo con metodología etnográfica sobre las intervenciones profesionales y las diferentes formas de intervención justificadas en esta noción en el contexto de internación de un hospital psiquiátrico de la Ciudad Autónoma de Buenos Aires, en Argentina. El análisis se orienta a visibilizar las tensiones morales que subyacen a las prácticas de intervención psicoanalíticas revisando su fundamentación teórica y aplicación clínica. A partir de la indagación pudo observarse que las intervenciones basadas en el concepto de responsabilidad subjetiva oscilan entre una orientación hacia la autonomía y la culpabilidad como dos caras de un proceso de individualización y privatización del sufrimiento que forma parte de un ethos cultural más amplio. Se observó una escasa consideración de los aspectos institucionales que forman parte de la vida de los pacientes y alteran la orientación de las intervenciones dirigidas a modificaciones individuales, así como la imposibilidad de evaluar si las intervenciones que se orientan a promover la �responsabilidad subjetiva� tienen eficacia en las respuestas de los pacientes, es decir si finalmente cumplen con su objetivo terapéutico.

    The aim of this work is to discuss the uses and practical implications of the psychoanalytic notion of �subjective responsibility� in a hospital setting. A qualitative study has been performed with an ethnographic methodology on the professional interventions and the different ways of intervening based on this notion in the context of internment in a psychiatric hospital in Ciudad Autonoma de Buenos Aires, Argentina. The analysis is oriented to give visibility to the underlying moral tensions in practices and psychoanalytic interventions as well as to review its theoretical basis and clinical application. From the inquiry, it could be observed that interventions based on the concept of �subjective responsibility� range between an orientation to autonomy and guilt as 2 faces of an individualization and privatization process of suffering that is part of a broader cultural ethos. It was observed that there was a minimun consideration of institutional aspects that are part of the patients� lives and that alter the orientation of interventions directed to individual changes, as well as the impossibility to evaluate if interventions oriented to promote �subjective responsibility� are efficacious in the patients´ responses, that is, if the therapeutic objective is achieved.
