Sueño amenazante en un grupo homogéneo de pacientes en espera de transplante renal

    El presente análisis es el resultado del trabajo con un grupo de pacientes dializados en espera de trasplante renal, realizado a fines de 1997 y comienzos de 1998. Con él pudimos apreciar que este grupo se encontraba en un fuerte estado regresivo, debido en parte a las diversas pérdidas de orden afectivo, partiendo por la primera gran pérdida, la función renal. Además, esta regresión se fomenta por la situación misma de diálisis, tanto por la institucional como por la relación con la máquina dializadora, dadora de vida o muerte, lo que hace revivir angustias primarias- descritas por Melanie Klein- como la lucha de por la vida y el temor a la muerte. Intentamos una mirada conjunta desde lo onírico y lo regresivo, partiendo del supuesto que el sueño es un estado regresivo. Así, pudimos ver la dinámica grupal como un fenómeno regresivo- onírico, en donde aparece lo amenazante expresado en la aterradora fantasía de "comerse el objeto donado", lo que nos indicaría que el grupo como un todo regresó a una fase oral canibalística. Desde ahí se pudo elaborar los duelos de este estado, en conjunto con el duelo de término de la terapia y de ese modo, poder lograr "otra identidad", que les permitiera vivir su particular situación, como "sujetos sufrientes" y no como seres que han perdido todo, hasta su identidad.

    The current analysis is the result of a study carried out in late 1997 and early 1998 with a group of dialyzed patients waiting for a kidney transplant. We could realize that this group was in a strong regressive state, due in part to the different emotional losses, beginning with the first big loss; the renal function. Besides, this regression is encouraged by the dialysis situation itself, for the institutional as much as the relation with the dialysis machine: life giving or taking what makes to revive the primary anguishes described by Melanie Klein , as the struggle for life and fear of death. We try a linked view from the oneiric to the regressive, starting from assumption that dreaming is a regressive state. Thus, we could see group dynamics as a regressive- oneiric phenomenon, in which the threatening is expressed in the fearful fantasy of "eating the given object" which would show us that the group as a whole went back to an oral cannibal stage. From there we could elaborate the mournings of this state together with the mourning of finishing the therapy. In this way they could get "another identy", wich could allow them to live their particular situation as "suffering individuals" and not as beings that have lost every thing, even their identy.
