Introduction: The association of VACTERL corresponds to 3 or more malformations: Vertebral, Anal, Cardiac, Tracheoesophageal, Renal and limb alterations (Lims). Its manifestation is associated with genetic and environmental factors, such as mutations in the HOXD13 and ZIC3 genes and also diseases such as gestational diabetes. Objective: To present a case of VACTERL association seen at the Santa Casa Hospital in Belo Horizonte. Materials and methods: This report concerns a 10-year-old female patient with oesophageal atresia, anorectal anomaly, hydronephrosis and sacral agenesis who was referred to the study hospital for an elective procedure involving bilateral ureteral reimplantation, Mitrofanoff appendicovesicostomy and vesicostomy closure. Conclusion: Despite the rarity of this pathology, if it is known by professionals and managed with expertise, a good quality of life is expected for affected patients. The prognosis, however, depends on the extent and severity of the associated anomalies, as well as the training of caregivers and the entire professional team.
