Modificaciones gasométricas, hematológicas e inmunológicas como indicadores de la biocompatibiidad de la membrana eval

    Es bien conocido que el contacto de la membrana de diálisis con la sangre del paciente puede dar lugar a reacciones que sean indicativas de la biocompatibilidad (BC) de dicha membrana. Hemos estudiado la BC de la membrana EVAL (etinilvinilzilcohol) en un grupo de diez pacientes con un total de 780 sesiones de dialisis, valorando parámetros clásicos (gasométricos y alteraciones hematologícas), y paramétros de reciente utilización como los marcadores de actividad inmunológica (C3. C4,OKT3, OKT4, OKT8). Las modificaciones más significativas la experimentaron los leucocitos totales y los neutrofilos con un importante descenso a los 15’, con discreta recuperación a los 30’ y prácticamente normsali7ados al final de la sesión. El descenso linfocitaxio no fué tan significativo con recuperación final.Se evidencia hipoxemia significativa que se mantiene hasta el final de la hemodiálisis. La fracción C3 experimenta un descenso significativo a los 15’ y 30’ correlacionandose con la leucopenia intradialitica.Comparativamente con el Cuprofan y el Poliacrilo nitrilo creemos que el EVAL ocupa una situación intermedia en cuanto a su biocompatibilidad.

    It is well known that contact between the dialysis membrane and the patient’s blood may cause reactions that indicate the biocompatibility (BC) of the said membrane . We have studied the BC of the EVAL membrane (Ethynilvinylalcohol) la a group of ten patients with a total of 780 sessions of dialysis, evaluating the usual paraineters (gasomefric and hematological alterations), and also recently used parameters such immunological activity markers (C3, C4,OKT3, OKT4 and OKT8). The most significant modifications are seen in the total leucocytes and the neutrophyls with an important descent at 15’, with discrete recovery after 30’ and practically a return tu normal at the end of the session. The lynphocyte descent was not so signiflcant related tu final recovery.Signiflcant hypoxemia was noted and this continued until the end of the haemodialysis.The C3 fraction undérwent a signiflcant descent at 15’ and 30’, correlated tu the intradialytical leucophene.Compared with Cuprofan and Polyacrylonitryl,we believe that EVALholds an intermediate position wlth regard tu ita biocompatibility.
