    Las hernias inguinales complejas (inguinoescrotales) o aquellas que no son reductibles espontáneamente y precisan reducción manual o las deslizadas o aquellas con incarceración crónica o subaguda son el talón de Aquiles de la hernioplastia laparoscópica totalmente extraperitoneal (TEP). En estas hernias, el desarrollo del espacio preperitoneal suele ser a la vez que insuficiente, reducido y además en general los repetidos intentos de reducción del saco herniario ocasionan desgarros peritoneales o lesión del contenido del mismo. Dado que en principio se considera el TEP como la técnica laparoscópica de elección, por ser “más fisiológica” que el TAPP, los intentos deben dirigirse a mantener como técnica de elección a la primera.

    Major inguinal hernias (inguinoscrotal) or those that are not spontaneously reducible and require manual reduction or slipped or those with chronic or subacute incarceration are the Achilles heel of the laparoscopic totally extraperitoneal hernioplasty (PET). In these hernias, the development of the preperitoneal space is usually both insufficient, reduced and, in general, the repeated attempts to reduce the hernial sac cause peritoneal tears or injury of the contents thereof. Given that in principle it is considered the PET as the laparoscopic technique of choice, since it is “more physiological” than the TAPP, the attempts must be directed to maintain the technique of choice as the first technique.
