Uno de los pilares fundamentales en la toma de decisiones en el anciano oncológico lo representa la valoración geriátrica integral (VGI). Este proceso multidimensional e interdisciplinar, y ya citado en el artículo previo, permite evaluar de forma fidedigna cuál es la situación basal del anciano con cáncer. Sin embargo, otro de los pilares fundamentales en dicha toma de decisiones en Oncogeriatría lo conforma el tumor en sí, es decir, sus características, su comportamiento y el pronóstico vital que le confiere al individuo. En este sentido resulta interesante conocer la relación que existe entre el envejecimiento y la aparición de cáncer, qué indicación tienen las técnicas de cribado para detectar precozmente determinados tumores en el anciano, cuáles son las peculiaridades de los mismos, y cómo predecir su pronóstico, y, por tanto, cómo estimar si existe beneficio con la administración de la quimioterapia adyuvante. Tal información es la que se pretende aportar con este artículo.

One of the fundamental pillars in decision-making in the oncological elderly is represented by the Comprehensive Geriatric Assessment (CGA). This multidimensional and in - terdisciplinary process makes it possible to reliably assess the baseline situation of the elderly with cancer. However, another of the fundamental pillars in the decision-making process in Geriatric Oncology is made up of the tumor itself, that is, its behavior and the vital prognosis that it confers on the individual. It is interesting to know the relationship between aging and the appearance of cancer, the interest of the early detection of tumours in older patients (screening tests), their differential characteristics, and how to predict their prognosis, and, therefore, how to estimate if there is or not benefit with the administration of antitumour treatment.
