  Las fistulas gastropleurales son un hallaz-go infrecuente debido a la barrera formada por el diafragma entre el estómago y la cavidad pleu-ral, siendo la mayoría causadas por traumatismo, neoplasia y cirugía. Presentamos el caso de un paciente de 63 años, diagnosticado de neoplasia gástrica (GIST) con afectación hepática. Acude por presentar dolor abdominal persistente de inten-sidad moderada-grave localizado en epigastrio y en hipocondrio izquierdo y malestar general. A la exploración física presenta dolor a la palpación con sensación de masa epigástrica. En la radio-grafía de tórax se aprecian signos de hidroneumo-tórax, por lo que se decide completar estudio con TC toraco-abdominopélvico con contraste, donde se confirma la presencia de fístula gastropleural izquierda segundaria a perforación gástrica por tumoración ya conocida

  Gastropleural fistulas are an infrequent fin-ding due to the barrier formed by the diaphragm between the stomach and the pleural cavity, the majority being caused by trauma, neoplasia, and surgery. We present the case of a 63-year-old pa-tient, diagnosed with gastric neoplasia (GIST) with liver involvement. He presented with persistent abdominal pain of moderate-severe intensity located in the epigastrium and in the left upper quadrant and general malaise. On physical exa-mination, she presented tenderness with a sensa-tion of an epigastric mass. The chest X-ray shows signs of hydropneumothorax, so it was decided to complete a study with a thoracic-abdomiopelvic CT with contrast, where the presence of a left gas-tropleural fistula secondary to gastric perforation due to an already known Cardiac-fundus tumor was confirmed.
