    La enfermedad neumocócica invasiva se de-fine como una infección confirmada por el aisla-miento de Streptococcus pneumoniae de un sitio normalmente estéril (sangre, líquido cefalorraquí-deo, pleura...) y que sobretodo afecta a edades ex-tremas de la vida (> 65 años y < 2 años). Dentro de dicho proceso, el Síndrome de Austrian engloba la triada de Osler: neumonía, meningitis y endocardi-tis, siendo ésta última más frecuente sobre válvula aórtica y la que determina el pronóstico habitual-mente. Presentamos el caso de una paciente que acudió a Urgencias con únicos síntomas de otalgia y fiebre, pero con evolución rápida y desarrollo del Síndrome de Austrian. A pesar de tratamiento ade-cuado desde el inicio, numerosas complicaciones a nivel neurológico fueron las que determinaron la evolución final.

    Invasive pneumococcal disease is defined as an infection confirmed by the isolation of Strep-tococcus pneumoniae from a normally sterile site (blood, cerebrospinal fluid, and pleural...) and which mostly affects extreme ages of life (>65 years old and < 2 years old). Within this pathology, Austrian Syndrome encompasses the Osler trial: pneumonia, meningitis and endocarditis. Aortic valve is the one more affected in endocarditis, and which usually determines the prognosis. We present a case of a patient who was incoming in the Emergency De-partment with otalgia and temperature over 38ºC, but with rapid evolution and development of Aus-trian Syndrome. Despite adequate treatment from the beginning, numerous complications at the neu-rological level were what determined the final evo-lution.
