Resumen de Complicaciones mecánicas en prótesis fijas implantosoportadas

  La prótesis fija sobre implantes es una opción terapéutica frecuente y previsible. Los problemas mecánicos de estas prótesis son las complicaciones más frecuentes en los tratamientos dentales con implantes.

  El objetivo del presente trabajo es estudiar las complicaciones mecánicas de las prótesis fijas sobre implantes colocadas en el Título Propio de Implantología Oral de la Universidad del País Vasco desde el año 2005 al 2010, así como analizar las complicaciones mecánicas de las diferentes casas comerciales y entre el tipo de restauración cementada o atornillada.

  Se diseñó un sistema de investigación clínico, observacional y transversal, también un estudio retrospectivo ya que recogimos información de la historia clínica del paciente.

  La muestra consistió en todos los pacientes de la población elegible a la que se le había colocado una prótesis fija sobre implantes entre el 1/1/2005 y el 31/12/2010. Se excluyeron las prótesis híbridas.

  En total, 60 pacientes acuden al estudio con un total de 171 implantes. El riesgo de complicaciones ha sido del 60%. La principal complicación encontrada ha sido la fractura de la porcelana con una incidencia acumulada del 30% y la descementación con un 22%. El aflojamiento del tornillo de la prótesis tiene una incidencia acumulada del 6% . El riesgo del resto de eventos es muy bajo. No existen diferencias significativas entre las dos casas comerciales incluidas en el estudio, Straumann y Nobel Biocare, en relación con el riesgo de tener una complicación chi-cuadrado 0,89 p=0,3467. Tampoco existe diferencia entre las prótesis atornilladas y cementadas: chi-cuadrado de 0,13 con una p de 0.7222 nos indica que el resultado no es significativo.

  Fixed partial dentures on implants is a common therapeutic option. Mechanical complications are the most common of all the problems that can be found in this type of prostheses.

  Study the mechanical complications of fixed dental prostheses on implants placed on the Postgrade of Oral Implantology of the University of the Basque Country. These implants were placed between 2005 and 2010. Analise the mechanic complications of the different implant brands and of the different types of restaurations that could be either cemented or screwed.

  A clinical, observational and transversal investigation study was designed. It is also considered a retrospective study because we obtained information from the case history of each patient.

  The sample consisted of all the patients that had a fixed partial prostheses from the years 2005 to 2010. Hybrid prostheses were excluded.

  60 patients took part of the study with a total of 171 implants. The risk of complications is of 60%. The main complication was the fracture of the porcelain with a cumulative incidence of 30%. The descementation had a cumulative incidence of 22% and the loosening of the screw had a 6% of incidence.

  The risk of the rest of events is very low. There are no significant differences between the two implant brands included on the stud, Straumann and Nobel Biocare, in relation to having a complication with a Chi squared of 0,89 and a p=0,3467. No difference was found between the screwed prostheses and the cemented prostheses with a Chi squared of 0,13 and a p of 0.7222 considering the result non significant.
