Objetivo. Conocer las características clínicas de los pacientes ingresados en una planta de Medicina Interna en situación de enfermedad crónica avanzada y con necesidades paliativas.
Método. Estudio observacional, transversal, descriptivo, desde el 15 de junio de 2010 al 15 de Julio de 2010; se incluyó exclusivamente a todos los pacientes ingresados en la planta séptima del Hospital Virgen de la Luz de Cuenca.
Resultados. Se incluyeron 104 pacientes (53,8 % varones, edad media 76,31 (DE 16,97); ingreso los 3 meses previos 25 %; mortalidad hospitalaria 6,7 %; institucionalizados 24 %; con cuidador principal 68,3 %; parentesco más frecuente pareja e hijos ambos 28,8 %. Cumplían criterios de enfermedad crónica avanzada 25 %. Karnofsky menor de 60 en 36,5% y 39,4 % con deterioro cognitivo moderado-grave. Se encontró correlación de la edad con el índice Karnofsky (r = -0,58, p< 0,001) y el cuestionario portátil del estado mental de Pfeiffer versión española SPMSQ-VE (r = 0,61, p< 0,001).
Conclusiones. Más de 50 % de los pacientes precisaban cuidador; un 25 % de los ingresados se encontraban en situación de enfermedad crónica avanzada.
Objetive. To describe the clinical characteristics of patient approaching the end of life admitted to an internal medicine ward.
Material and methods. An observational, descriptive cross-sectional study, by clinical interview, from June 15, 2010 to July 15, 2010 to all patients admitted to one of the Virgen de la Luz Hospital´s ward.
Results. A total of 104 patients were included. 53.8% were male. The mean age was 76.31 ± 16.97. Had submitted prior income during the previous 3 months 25%. were readmitted in the post-month high home 4.8%. Hospital mortality was 6.7%. Home was the residence housing 24%. Had a primary caregiver 68.3%, of these, the most common relationship was the spouse and son both with a frequency of 28.8%. 25% met terminal criteria. 36.5% of patients had a Karnofsky less than 60 and 39.4% had moderate to severe cognitive impairment. A correlation was found between age and Karnofsky index (r = -0.58, p < 0.001) as well as the age and the questionnaire results (short mental status questionnaire spanish version) SPMSQ-VE (r = 0.61, p < 0.001).
Conclusion. Up to 50 % of the patients needed caregiver and up to 25 % of those admitted patients were approaching the end of life.
