Monitorización domiciliaria a pacientes portadores de desfibriladores automáticos implantables: Experiencia de enfermería

    • Introducción: Se analizó la experiencia de dos hospitales de la Comunidad Andaluza, el Hospital Universitario Reina Sofía, de Córdoba y el Hospital Universitario Virgen de Valme, de Sevilla, con la nueva plataforma tecnoló- gica CareLink que permite acceder de forma remota a los datos almacenados en los Desfibriladores Automáticos Implantables (DAI).

    • Objetivo: Se pretendió valorar si el programa educativo, impartido por el equipo de enfermería, de ambos Hospitales, fue beneficioso para los pacientes, y el grado de satisfacción/confianza con el sistema de los mismos.

    • Material y métodos: Se incluyeron 20 pacientes, que recibieron una completa formación del sistema por parte de nuestro equipo, así como materiales educacionales (DVD y documentación). Se realizó una primera transmisión manual desde el propio Hospital, y se les solicitó que realizaran transmisiones desde casa cada 15 días.

    • Discusión y conclusiones: El 100% de la información fue obtenida de forma correcta. Los pacientes cumplieron rigurosamente el calendario de transmisiones programadas y manifestaron mediante un cuestionario, un alto grado de satisfacción con el entrenamiento recibido, así cómo con la seguridad y confianza que les proporcionaba el sistema.

    • Introduction: Two hospitals of the Andalusia, in the south of Spain (Europe), the Universitary Hospital “Reina Sofía” of Cordoba and the Universitary Hospital “Virgen de Valme” of Seville where analysed, they have a CareLink technological platform which allows us to remotely access those data stored in the Implantable Automatic Defibrillators (IAD), not being necessary for the patient to visit the Hospital.

    • Aims: To analyse our experience with these devices, to assess whether our training programme has been beneficial and to evaluate the degree of satisfaction of the patients regarding the monitor use.

    • Material and method: Descriptive and prospective research with 20 patients who use this technology. At home, they interrogate their implantable device through the monitor, transmitting the information to a secure server by means of a standard analogical phone line. For reviewing those data, at Doctor’s office, just a computer with an Internet connection is necessary.

    • Discussion and conclusions: 100% of the information transmissions have been correct; all patients have complied with the schedule regarding the remote follow ups which had been previously agreed on our office, and the patients have expressed, through a questionnaire, a high degree of satisfaction regarding this programme, and deem our training system to be very positive.

    This system allows a remote monitoring of the implanted devices. This is very useful for our patients, as a quick transmission from their homes can provide us with the necessary information to be able to diagnose any problems and to decide whether the patient should visit or not the Hospital. A complete training by the nursing staff helps the patient, preventing any groundless concern or any unnecessary visit to our Hospital.
