Evaluación de la calidad de las guías de práctica clínica de artritis reumatoide con recomendaciones sobre la salud del pie

    Introducción.

    La deformidad y el dolor del pie son casi omnipresentes (90%) en la artritis reumatoide (AR), debido a la interacción entre inflamación y biomecánica anormal. Las intervenciones no farmacológicas (ortesis, calzado) tienen un papel importante pero permanecen siendo un área descuidada. Las guías de práctica clínica (GPC) tienen recomendaciones para el cuidado del paciente, que consiguen disminuir la variabilidad en la práctica clínica. Hay varias GPC generales para tratar a pacientes con AR, pero no para la patología del pie. El objetivo de este trabajo es desarrollar GPC que cubran el tratamiento no farmacológico de la patología del pie en pacientes con AR.

    Método.

    Revisión de la literatura científica para identificar todas las GPC sobre la AR y el pie, entre 1975-2016. El instrumento usado para evaluar las GPC fue el AGREE II.

    Resultados.

    Con respecto a la calidad global, la mejor GPC es la NICE (puntuación 6/7) siendo las guías: GUIPCAR y Brazilian Society of Rheumatology Consensus for the treatment of rheumatoid arthritis, las que tienen las puntuación más bajas. En relación a las recomendaciones del pie, no en todas las GPC aparecen, la guía Brazilian Society of Rheumatology Consensus for the treatment of rheumatoid arthritis no presenta, siendo las guías que tienen más la HAS y la Guidelines for the management of the foot health problems associated with rheumatoid arthritis.

    Conclusión.

    La guía de AR general más recomendable es la guía NICE. Hay además otras tres GPC recomendables: la guía RACGP, la guía SIGN, y la guía Non-drug treatment (excluding surgery) in rheumatoid arthritis.

    Introduction.

    Deformity and foot pain are almost omnipresent (90%) in rheumatoid arthritis (RA), due to the interaction between inflammation and abnormal biomechanical. Non-pharmacological interventions (insoles, footwear) have an important role but it is still a careless area. Clinical practice guidelines (CPG) have recommendations for patient care, reducing variability in clinical practice. There are several general CPG to treat patients with RA, but not for foot pathology. The aim of this work is to develop CPG meeting the demand of non-pharmacological treatment of foot pathology in patients with RA.

    Method.

    Review of the scientific literature to identify all CPG of RA and foot, between 1975-2016. The instrument used to evaluate the CPG was the AGREE II.

    Results.

    In relation to the overall quality, the best CPG is NICE CPG (score 6/7) and GUIPCAR and Brazilian Society of Rheumatology Consensus for the treatment of rheumatoid arthritis CPG have the lowest score. In relation to the recommendations of the foot, not all CPG have them (Brazilian Society of Rheumatology Consensus for the treatment of rheumatoid arthritis CPG no) and those with more are the HAS and Guidelines for the management of the foot health problems associated with rheumatoid arthritis CPG.
