Infecciones parotídeas supuradas de distinto origen

    Los abscesos parotídeos son complicaciones raras de las parotiditis agudas. Los agentes patógenos más frecuentes sociados a infecciones agudas son el estreptococo, el estafilococo áureus y las bacterias anaerobias. Generalmente requieren tratamiento antibiótico intravenoso y drenaje quirúrgico. Presentamos tres casos completamente diferentes en cuanto a etiología y forma de tratamiento. El primer paciente sufrió una parotiditis recidivante abscesificada que respondió bien a drenajes repetidos bajo anestesia local. Nuestro segundo caso debutó como una parotiditis aguda. Los drenajes repetidos llevaron a la formación de una ístula, siendo necesaria la parotidectomía superficial. En este caso el origen del absceso fue un quiste parotídeo sobreinfectado. Nuestro tercer paciente presentó un absceso secundario a una infección amigdalar. El material purulento atravesó las fascias profundas del cuello hacia el espacio preestíleo laterofaríngeo y la región parotídea a continuación.

    Parotid abscesses are rare complications of acute suppurative parotitis. The most common pathogens associated with acute bacterial infection are Staphylococcus aureus, Streptococcus and anaerobic bacteria. They usually require intravenous antibiotic therapy and surgical drainage. We report three cases completely different in their ethiology and treatment. The first one was a recurrent parotideal abscess that recovered well with repeated surgical drainage under local anesthesia. Our second patient appeared as an a cute parotitis, but repeated surgical drainage led to fistula formation, being necessary superficial parotidectomy. In this case the origin of the abscess was an infected parotid cyst. Our third patient presented with an abscess following a tonsillar infection. The purulent material spread trough the fascial neck spaces towards the lateropharyngeal preestileo space and the parotid gland.
