Abstract
The Knee Total Prosthesis (KTP) has been proved to
be an efficient procedure when
treating pain and physical disability [36, 170], im
proving the quality of life
[80,139,117].
Aspects like mental health or the concept of distre
ss have gained importance [60] in
medical literature among the factors influencing th
e KTP results. The concept of
distress comes from the distinction made by Hans Se
yle (1974) between positive stress
“eustres” and negative stress “distress”. When the
response is adjusted to the needs
provided by the stimulus or aggression, eustres or
physiological adaptation occurs. On
the contrary, if the response is insufficient, wron
g or excessive, distress appears [150].
Many have been the instruments used to measure dist
ress: HADS [33, 35], Brief
Symptom Inventory [43], SCL-90 [42], GHQ [64, 105],
Distress Thermometer [123]
and MH Score (mental health subscale SF-36) [167].
The analysis of distress is difficult
due to this huge variety of questionnaires for the
study.
In this project, HADS [173] was used to make psychi
c distress objective in situations of
anxiety and depression. In order to assess the resu
lts of the KTP from a medical point of
view, the Knee Society Score [5, 85, 92] was used,
and the WOMAC [1, 5, 145, 15, 52,
58, 32, 121, 127, 28, 51, 135, 169], for the qualit
y of life. La Prótesis Total de Rodilla (PTR) ha demostrado ser un procedimiento de eficacia probada en el tratamiento del dolor y la incapacidad física [36, 170], considerándose un instrumento que mejora la calidad de vida [80, 139, 117].
Entre los factores que influyen en los resultados de la PTR, aspectos como la salud mental han cobrado relevancia [60], y un concepto como el distrés ha ganado notoriedad en la literatura médica.
El concepto distres proviene de la distinción que realiza, HANS SELYE (1974), entre estrés positivo o “eustrés” y estrés negativo o “distrés”. Cuando la respuesta se ajusta alas necesidades generadas por el estímulo o agresión, se produce una adaptación fisiológica o eustrés; por el contrario, si la respuesta es insuficiente, errónea o excesiva, se produce un distrés [150].
Numerosos han sido los instrumentos utilizados para medir distrés:
HADS [33,35],
Brief Sympton Inventory [43], SCL-90 [42],
GHQ [64,105], Distress Thermometer [123],
MH Score (subescala salud mental SF-36) [167].
Esta gran diversidad de cuestionarios para el estudio del distrés hace difícil su análisis.
En este trabajo se utilizó el HADS [173], para objetivar distrés psíquico en sus
manifestaciones de ansiedad y depresión. Para valorar los resultados de la prótesis total de rodilla desde el punto de vista médico se utilizó el Knee Society Score [5, 85, 92], y para la calidad de vida el WOMAC [5, 145, 1, 15, 52, 58, 32, 121, 127, 28, 51, 135, 169].
