Eficacia de un terapia cognitivo conductural(TCC) y de la TCC-focalizada en el trauma en infancia maltratada.

    • Objetivo: El propósito fundamental del presente trabajo fue proporcionar evidencia empírica acerca de la eficacia del modelo de Terapia Cognitivo Conductual (TCC) en general, y del modelo de TCC-Focalizada en el Trauma (TCC-FT) en particular, para el tratamiento del estrés postraumático, la sintomatología depresiva y los problemas del comportamiento, en niños y adolescentes víctimas de maltrato intrafamiliar crónico en situación de acogimiento residencial o familiar. Para ello, se llevó a cabo el análisis de la eficacia de tres protocolos de tratamiento basados en el TCC; uno de ellos, Focalizado en las Emociones (PEDIMET-FE); y otros dos, basados en el modelo TCC-FT en su versión completa y abreviada (PEDIMET-FT y PEDIMET-FT-ABR). Todos ellos con la inclusión de la Realidad Virtual. Método: La muestra final estuvo compuesta por 46 menores con edades comprendidas entre 6 y 16 años (52.2% niños y 47.8% niñas), de los cuales el 67.4% se encontraban en situación de acogimiento residencial y el 32.6% en acogimiento familiar. Los instrumentos de evaluación de la sintomatología empleados fueron: el CPSS (Child PTSD Symptoms Scale; Foa et al., 2001) para detectar la sintomatología de estrés postraumático; el CDI (Cuestionario de Depresión Infantil; Kovacs, 1992) para la sintomatología depresiva; y el CBCL (Achenbach y Edelbrock, 1983) para evaluar los problemas del comportamiento. La eficacia se analizó mediante la prueba "t" de Student y el índice del tamaño del efecto de la diferencia media tipificada (d). Resultados: los resultados mostraron diferencias estadísticamente significativas y tamaños del efecto grandes y moderados en las tres condiciones de tratamiento PEDIMET para los indicadores de estrés postraumático (reexperimentación, evitación e hiperactivación), depresión global (disforia y autoestima) y problemas del comportamiento internalizante y externalizante. Los modelos de PEDIMET-Focalizados en el Trauma, basados en el modelo original de TCC-FT (Cohen, Deblinger y Mannarino, 2006), especialmente el PEDIMET-FT-ABR, han mostrado beneficios muy destacables en toda la sintomatología evaluada. El modelo PEDIMET-Focalizada en Emociones ha mostrado su eficacia principalmente en la reducción de todos los indicadores de estrés postraumático, la disforia, ansiedad/depresión y los problemas externalizantes. Conclusiones: se ha cumplido el objetivo general planteado en la presente investigación, esto es, proporcionar evidencia empírica de la eficacia de los protocolos de tratamiento PEDIMET (PEDIMET-FT, PEDIMET-FT-ABR y PEDIMET-FE) dirigidos a mejorar la salud mental de los menores víctimas de maltrato intrafamiliar. Así la presente investigación se convierte en el primer estudio empírico en España que examina y confirma la eficacia de las terapias TCC y TCC-FT en el tratamiento psicológico de niños, niñas y adolescentes víctimas de maltrato infantil.

      Objective: The main purpose of this study was to provide empirical evidence about the efficacy of the model of cognitive behavioral therapy (CBT) model in general and TCC-Focused Trauma (TCC-FT) in particular for the treatment of stress posttraumatic, depressive symptoms and behavioral problems in children and adolescents victims of chronic domestic abuse in situations of residential or foster care. To this, it was performed analyzing the effectiveness of three treatment protocols based on the TCC; one of them on Emotions Focused (PEDIMET-EF); and two others, based on the TCC-FT model in full and brief version (PEDIMET-TF and B-PEDIMET-TF). All with the inclusion of Virtual Reality. Method: The final sample consisted of 46 children aged between 6 and 16 years (52.2% boys and 47.8% girls), of which 67.4% were in a situation of residential child care and 32.6 % in foster care. The assessment instruments used were: the CPSS (Child PTSD Symptoms Scale; Foa et al., 2001) to detect the symptoms of post-traumatic stress; the CDI (Children's Depression Questionnaire; Kovacs, 1992) for depressive symptoms; and CBCL (Achenbach and Edelbrock, 1983) to assess behavioral problems. Efficacy was analyzed using the "t" Student test and effect size index of the standardized mean difference (d). Results: The results showed statistically significant differences and large and moderate effect sizes in all three treatment conditions PEDIMET for indicators of PTSD (re-experiencing, avoidance and hyperarousal), global depression (dysphoria and self-esteem) and internalizing and externalizing behavior problems. PEDIMET- Trauma Focused models, based on the original model TCC-FT (Cohen and Mannarino Deblinger, 2006), especially brief version of PEDIMET-FT, have shown extraordinary benefits throughout the symptoms evaluated. The PEDIMET-Focused Emotions has been effective primarily on reducing all indicators of PTSD, dysphoria, anxiety/depression and externalizing problems. Conclusions: In sum, it has met the overall objective in this investigation, that is, to prove empirical evidence of the effectiveness of PEDIMET treatment protocols (PEDIMET-FT, B-PEDIMET-FT and PEDIMET-EF) on improving the mental health of abused children. So this research becomes the first empirical study in Spain examining the effectiveness of CBT and CBT-TF in the treatment of children and adolescents who have been victims of child abuse.
