Patrones de respuesta a la inmunoterapia en el melanoma metastásico

    La inmunoterapia se ha establecido en muy poco tiempo como uno de los pilares fundamentales del tratamiento del cáncer. En melanoma fue en el primer tumor en donde se observó beneficio y también unas características especiales de respuesta.

    Esta respuesta diferencial se debe al mecanismo de acción que hace llegar las células del sistema inmune al tumor, pudiéndose observar latencia en el comienzo de la acción y, en ocasiones, crecimiento de las lesiones. Además, había pacientes que no respondían de forma objetiva, pero se convertían en largos supervivientes con la estabilización de las lesiones. A partir de las observaciones de los ensayos clínicos iniciales con ipilimumab, se han definido unos criterios de respuesta inmunorrelaccionados, que permiten tomar decisiones acerca de la necesidad de continuar el tratamiento en pacientes con respuestas atípicas.

    Immunotherapy has become a mayor pillar in cancer treatment in a relatively short time. Malignant melanoma was the first tumor to benefit from this treatment and also the first in which we observed special patterns of response.

    This different response was attributable to the immunotherapy mechanism of action that requires the immune cells to reach the tumor, this can cause a delay in the starting of the effect and also an increase in the size of lesions. There were also patients without a conventional response that became long term survivors. According to these observations and with data form the early ipilimumab trials immune related response criteria have been developed as an aid in taking therapeutic decisions in patient with atypical response patterns.
