Objective: To describe the development of a computer tool for reconciling medication, as well as its impact on the reduction of unjustified discrepancies. Method: The computer tool was developed with the programming language JAVA 1.6. The software used for its development was NETBEANS 6.9 and the database was FIREBIRD 1.5.6. In order to evaluate this tool, we performed a quasi-experimental pilot study. Results: The computer tool for reconciliation is integrated in the electronic prescribing program. It allows recording the pharmacotherapeutic records and the automation of medications for the patient's active prescription. After implementing the tool, the proportion of unjustified discrepancies significantly decreased from 7.2% to 4.2%, p <0.05.
