Nivel de apego de los registros clínicos de enfermería a la Norma Oficial Mexicana NOM-168-SSA1-1998, del Expediente Clínico

    Introducción. Los registros de enfermería son el soporte documental de la atención médica, en los que se plasma la información acerca de las diversas actividades que se realizan en el paciente y hacen posible el intercambio de información sobre los cuidados, promueven la comunicación entre enfermería con otros miembros del equipo de salud y aportan elementos para evaluar el proceso de atención. Esta investigación se orientó a identificar el nivel de apego de los registros clínicos de enfermería a la Norma Oficial Mexicana NOM-168-SSA1-1998, del Expediente Clínico (NOM 168).

    Material y Métodos. Se efectuó un estudio descriptivo, transversal y retrospectivo, evaluándose 257 registros de enfermería de expedientes clínicos utilizados en el procedimiento arbitral de la Comisión Nacional de Arbitraje Médico (CONAMED), durante el año 2009. Se diseñó y aplicó un instrumento dicotómico basado en la NOM 168. Los datos fueron procesados empleando el paquete estadístico SPSS V.15.0 y se realizó análisis estadístico e inferencial.

    Resultados. El índice general del nivel de apego correspondió al 73.93% de los casos. Se identificaron deficiencias en los registros y el mayor porcentaje de incumplimiento consistió en la omisión de nombre completo y firma del personal de enfermería, así como de los datos relacionados con la descripción del habitus exterior. Los datos que con mayor frecuencia se registraron como indica la norma, fueron: signos vitales, nombre completo del paciente y anotaciones correspondientes a la administración de medicamentos.

    Conclusiones: Se establece que el personal de enfermería está obligado a conocer y atender las reglas generales de su profesión, tomando en cuenta que la NOM 168 determina los lineamientos específicos que deben cumplirse debido a su carácter técnico y obligatorio.

    Introduction. Infirmary registries are of great importance in the medical attention, as they constitute the documentary support in which the information about the diverse activities that are carried out in relation to the patient is recorded.

    These facilitate the exchange of information among carers, promoting the communication between infirmary personnel and other members of the healthcare team, at the same time, they provide elements to facilitate the evaluation eof the healthcare process. This research was oriented to identify the level of compliance of infirmary clinical registries with theo Mexican Clinical File (NOM 168) Official Norm, NOM- 168-SSA1-1998.

    Material and Methods. Descriptive, cross-sectional and retrospective study, where infirmary registries from clinical files used in arbitration procedures at the National Commission for Medical Arbitration (CONAMED) during 2009 were evaluated. A dichotomic instrument, based on the NOM-168.

    SSA1-1998, was designed and applied to the clinical records Data was processed using the statistical package SPSS V.15.0, performing statistic and inferential analysis.

    Results. The compliance general index was significantly acceptable in 73.93% of the cases. Several deficiencies were identified; the most persistent lack of compliance related to the omission of complete name and signature of infirmary personnel, as well as the lack of data related to the description of the patients' observed external appearancer. Data that most frequently was registered according to the NOM included: vital signs, patients' complete name and annotations regarding the administration of drugs.

    Conclusions. Infirmary personnel is bound to know and attend the general rules of their profession, taking into account that the NOM-168-SSA1-1998, of the Clinical File, establishes the specific lineaments that must be fulfilled due to their technical and obligatory character.
