Resumen de Efectividad de un tratamiento psicológico para dependientes a cocaina en atención primaria

    El objetivo de este estudio fue analizar la viabilidad y la efectividad del Programa de Reforzamiento Comunitario (CRA) en un grupo de adictos a la cocaína y comprobar qué variables pronosticaban mejor la retención en tratamiento. Se realizó en la Unidad de Conductas Adictivas de Catarroja (Departamento 10 de la Consellería de Sanitat de Valencia).

    Participaron 26 pacientes con diagnóstico de dependencia a la cocaína, a los que se les aplicó el CRA, con las adaptaciones necesarias para poder ser aplicado en una UCA de atención primaria. Se realizaron mediciones pre-post (seis meses) con EuropASI, MAST, SCL-90-R y BDI, utilizando contraste de Wilconxon. La abstinencia se evaluó con el test Multidrog-cocaína. Para pronosticar la retención en el tratamiento se realizó un análisis chaid y un análisis de regresión logística.

    La retención a los seis meses fue del 61,5% y la abstinencia continuada del 26,9%.

    Hubo mejoras en todas las áreas de funcionamiento, con significación estadística en los indicadores de alcohol, drogas, familiar/social y psiquiátrico del EuropASI; en el Índice de Severidad Global del SCL-90-R, y en el MAST. La variable que mejor pronosticaba la retención fue el número de controles analíticos negativos, con una estimación de riesgo de 0,079. Los datos de retención y abstinencia son similares a los obtenidos por este tipo de programa en otros ensayos previos. Se sugiere que el CRA es, con algunas adaptaciones, un programa viable para utilizar en un contexto de tratamiento público, y ofrece resultados positivos con una buena relación coste-beneficio.

    The objective of this study was to analyze the viability and effectiveness of a Community Reinforcement Approach (CRA) program in a group of cocaine addicts, and to identify which variables best predict treatment retention. Twenty-six patients diagnosed as cocaine dependent participed in Catarroja Addictive Behaviours Unit (Department 10 of the Valencia Health Service), application of the CRA program, with the necessary adaptation for use in a primary care Addictive Behaviours Unit. Principal pre-post measures (six months) with the EuropASI, MAST, SCL-90-R and BDI, applying the Wilcoxon test. Abstinence was assessed with the Multidrog-cocaine test. For predicting treatment retention a chaid analysis and a logistical regression analysis were carried out.

    The figure for retention at six months was 61.5%, and for continuous abstinence, 26.9%.

    There were improvements in all areas of functioning, with statistical significance in the alcohol, drugs, family/social and psychiatric indicators of the EuropASI; in the Global Severity Index of the SCL-90-R; and in the MAST. The variable that best predicted retention was number of negative tests, with a risk estimation of 0.079. Retention and abstinence data are similar to those obtained with this type of program in previous trials. They suggest that CRA is, with some adaptations, a viable program for use in a public healthcare treatment context, and obtains positive results with a good cost-benefit relationship.
