Factores emocionales, cognitivos y conductuales en la vivencia de la fibromialgia

      La fibromialgia (FM) es un síndrome de dolor crónico pobremente entendido. Diversos investigadores apuntan a una posible relación del curso y cronificación con alta susceptibilidad al estrés; pero falta claridad sobre que variables personales intervienen en la alta susceptibilidad al mismo frente a eventos cotidianos. Se toma como referente la evaluación cognitiva a que hace referencia la teoría transaccional del estrés. El objetivo fue identificar que variables situacionales y psicológicas influyen en la mayor vulnerabilidad al mismo.

      METODOLOGÍA:

      Estudio cualitativo con teoría fundamentada, realizado en mujeres con diagnóstico médico de FM según pautas del Colegio Americano de Reumatología. El parámetro para el tamaño muestral fue la saturación de información. Se aplica un cuestionario sociodemográfico y clínico, tests psicológicos sobre emociones, creencias nivel funcional y calidad de vida (STAXI-2, TCI-R, CTI, FIQ, COOP WONKA)y se realizan entrevistas en profundidad. Como criterio de calidad, para la confiabilidad se utilizo la triangulación de datos con un mínimo del 85% de acuerdos. Para la validez la discusión de expertos, diferentes fuentes de información y la persistencia de los conceptos y relaciones entre las diferentes participantes. El programa informático atlas.ti se utilizó como instrumento de análisis.

      RESULTADOS Y DISCUSIÓN:

      Se estudian 10 mujeres entre 30 y 63 años, con diagnóstico de FM. Estas valoraron tener bajo apoyo social relacionado con una alteración de tipo funcional poco reconocida socialmente. Situación que aumenta la susceptibilidad al estrés físico y emocional. Revelan estructura de valores con predominio de tradición y benevolencia. La estructura de valores de este grupo difiere con las personas sanas reportado en otros estudios. Las creencias sobre sí mismas se revelan en la misma dirección de sus valores. Hay una resistencia a modificar esas estructuras cognitivas las que entran en conflicto con las condiciones de limitación física, cognitiva y sociales de la FM. Por eso sus creencias y valores se constituyen en la principal fuente de estrés físico y psicológico.

      Predominan en su vivencia con la FM las emociones estresantes, las principales son ira, tristeza, miedo, culpa, vergüenza; indicando significados de humillación, perdida, autorreproche, fracaso en su identidad social. La experiencia de estrés es mayor en las más jóvenes por la evaluación de mayores demandas sociales y personales.

      CONCLUSIONES Es importante trabajar en la flexibilización de las expectativas definidas por la estructura de sus valores y por creencias. La educación a la comunidad, las afectadas y su familia sobre aspectos de la FM y modificar factores del medio social, pueden ser caminos para disminuir la susceptibilidad al estrés y cronificación de la FM.

      Fibromyalgia (FM) is a syndrome of chronic pain poorly understood. Several researchers point at a possible relation of the course and origin with high susceptibility to stress; but lack clarity about what personal variable take part in the high susceptibility to it front to daily events. It takes as reference the cognitive assessment, referred to the theory transactional of stress. The aim was to identify social and psychological factors that influence in the vulnerability to stress.

      METHODOLOGY: Qualitative study with grounded theory, conducted in women diagnosed with medical FM according patterns American College of Rheumatology. The parameter for the sample size was information saturation.

      Interviews are made in depth and a socio-demographic and clinical questionnaire is applied, evaluate psychological variables, functional level and quality of life (STAXI-2, TCI-R, CTI, COOP WONKA, FIQ). As a criterion for quality was taken into account, for reliability were used triangulation of data with a minimum of 85% of agreements. For the validity: the experts discussion, different sources of information and the persistence of concepts and relationships between the various participants., among others. The computer science program, atlas.ti, was used like analysis instrument.

      RESULTS AND DISCUSSION:

      We studied 10 women between 30 and 63 years, diagnosed with FM. They assessed as poor the social support, related to an alteration of functional type, little socially recognized. This situation increases the susceptibility to the physical and emotional stress.

      They reveal a structure of values with tradition and benevolence predominance. The structure of values of this group differs with healthy individuals reported in other studies. The beliefs on themselves are revealed in the same direction of their values.

      There is a resistance to change these cognitive structures which enter conflict with the terms of limiting physical, cognitive and social aspects of the FM.

      For that reason their beliefs and values are constituted in the main source of physical and psychological stress.

      The stressful emotions predominate in their experience with the FM, of these the main ones are wrath, sadness, fear, blames, shame; indicating meant of humiliation, lost, self-reproach, and failure in their social identity. The stress experience is greater in youngest by the evaluation of greater social and personal demands.

      CONCLUSIONS:

      It is important to work in the modification of the expectations defined by the structure of their values and beliefs. The education to the community, affected and its family on aspects of the FM, and modify factors of social environment, can be ways to diminish the susceptibility to chronic stress and worse clinical condition.
