Grupos de uso de recursos (RUG III) y niveles asistenciales en Geriatría

      OBJETIVOS: Aplicar el sistema de clasificación de pacientes RUG III, en varios centros y servicios de atención geriátrica con diferencias administrativas, de objetivos asistenciales y de ubicación. Evaluar la fiabilidad del sistema, calcular la carga asistencial, los índices Case-Mix y la relación entre índice de AVD y carga asistencial. Asimismo valorar si estas diferencias permiten una comprensión de los distintos niveles asistenciales. PACIENTES Y MÉTODOS: Se incluyeron cinco tipos de centros o servicios. Para cada uno se calculó el índice de AVD, la patología predominante, su inclusión en alguna de los 44 grupos RUG y el tiempo de dedicación del personal. Se obtuvieron, el ICM de cada centro o servicio, la fiabilidad del sistema mediante el índice Kappa; se evaluaron las diferencias de ICM y los grupos predominantes en cada centro y servicio, y por último la relación entre índice de AVD y carga asistencial mediante diagramas de dispersión, regresión y correlación. RESULTADOS: Se evaluaron un total de 509 pacientes. Los valores 4 (37,1%) y 18 (14,3%) en el índice de AVD, fueron los más representados, siendo la media 9. Los grupos encontrados con más frecuencia fueron: PA1 (22,1%), CA1 (6,5%), CC1 (6,5%) y SSC, PD1, PE1 con un 5,7%. La carga asistencial media fue de 100,3 minutos. Se encontró un índice de variación inferior al 50% en el 86,5% de los grupos encontrados. El índice de concordancia fue del 58,8%. El análisis de regresión permite explicar que el 88% de la variabilidad de la carga asistencial viene determinada por el índice de AVD. Comparando los centros y servicios se observa una relación entre índice de AVD, carga asistencial y grupos RUG, de menor a mayor a medida que aumenta la complejidad de los objetivos de los diferentes niveles asistenciales. CONCLUSIONES: En la muestra estudiada se encontraron casi todos los grupos del sistema RUG III, estando más representados en las residencias los grupos de menor complejidad y en los servicios con mas objetivos asistenciales, los más complejos. El índice de AVD explica la mayor parte de la carga asistencial. El sistema se mostró fiable y ayuda a comprender las diferencias entre los distintos niveles asistenciales.

      OBJECTIVES: To apply the classification system of RUG patients in several centres and services of geriatric assistance with different administrative, assistential objectives and location. To evalue the reliability of the system, to work out the assistential charge, the case-mix index (CMI) and the relation between DLA and the assistential charge. Likewise to evaluate if these differences allow a comprehension of the different assistential levels. PATIENTS AND METHODS: Five types of centres or services were included. In each one of them was calculated the DLA index, the prevailing pathology, there inclusion in any of the 44 RUG groups and time of personal dedication. The CMI of each service was calculated, the reliability of the system by the Kappa index, the differences of CMI and the prevailing groups in each centre or service, and also the relation between the DLA index and the assistential charge trough dispersion, regression and correlation diagrams. RESULTS: 509 patients were evaluated. The values 4 (37.1%) and 8 (14.3%) in the DLA index were the most represented, with an average of 9. The most frequent groups were PA1 (22.1%), CA1 (6.5%), CC1 (6.5%) and SSC, PD1, PE1 (5.7%). The average assistential charge was 100.3 minutes. The variation index was less than 50% in 86.5% of the groups. The concordance index was 58.8%. Through the regression analysis we can explain that 88% of the variability of the assistance charge is determined by DLA index. Comparing the centres and services a relation between the DLA index, the assistance charge and the RUG groups can be observed. This increases the complexity of the objectives of the different assistential levels. CONCLUSSIONS: In the studied sample were included almost all the groups of the RUG III system, being represented in the residences the less complex groups and the more complex in the services with more assistential objectives. The DLA index explains most of the assistential charge. The system proved to be reliable and helps to understand the differences between the different assistential levels.
