Conclusiones de los autores del estudio: las intervenciones realizadas en urgencias permiten incrementar la detección del número de casos sospechosos de abusos, pero la metodología de los estudios no permite extraer conclusiones acerca de su eficacia.
No se puede confirmar que la edad, la consulta repetida a urgencias y el tipo de lesiones permitan identificar a los niños con un posible maltrato o negligencia.
Comentario de los revisores: sin olvidar que las dos revisiones sistemáticas presentan limitaciones por la calidad de los artículos incluidos, por el momento no disponemos de una herramienta clínica para usar en urgencias que incremente la sospecha de maltrato; otros factores como la edad, tipo de lesiones y la repetición de visitas a urgencias se deben emplear con prudencia en el índice de sospecha ya que no se han relacionado con el maltrato infantil.
Authors� conclusions: emergency interventions can increase the detection of the number of suspected cases of abuse, but the methodology of the studies precludes drawing conclusions about their effectiveness. It cannot be confirmed that age, repeated attendance to the emergency department and type of injury identify children with suspected child abuse or neglect.
Rewiewers� commentary: both systematic reviews have limitations due to the quality of the articles. For the moment, we do not have a clinical tool to be used in the emergency department to increase the suspicion of abuse; other factors such as age, type of injury and repeated visits to the emergencies should be used with caution in the index of suspicion because they have not been found to be related to child abuse.
